Regarding attire - Informal casual wear is the norm at the tourist oriented restaurants.
Regarding tipping - 15% to 20% is the norm at restaurants with table service. Caution! A few unscrupulous wait-persons have double-swiped tourist credit-cards to add an exhorbitant/unauthorized big tip. If the wait-person asks the very common question "So where are you from?" you may have a reason to be suspicious as they like to target out-of-towners. You may want to be there when they swipe the card and never sign 2 receipts.
